This offering is for the party of completing this form and no fees will be associated with and collected by Patriot Pursuit inc. for the awarded trip. All fees agreed upon will be your direct payment to the licensing state of which you will be partaking in. Your travel expenses will be on your behalf unless discussed and agreed upon by both parties. Upon arrival Patriot Pursuit will cover all costs inherited that are previously stated and notified of you. Additional costs while on your trip for “extras” will not be covered by Patriot Pursuit inc. (add on animals, additional Tags, gratuity, alcohol and others). The client also recognizes that any cancellation and costs due to that reason will not be reimbursed by Patriot Pursuit inc. In the event Patriot Pursuit inc. and the associated outfitters are unable to perform any portion of their obligation hereunder for any reasons beyond the control of said organizations, including but not limited to weather, fire, acts of nature or governmental action, Client shall not be entitled to a refund of any portion of his / her previously paid fees associated. DISCLOSURE DISCLAIMER: Each Client and all members of their hunting or fishing party must read and initial the following: I UNDERSTAND AND AGREE that hunting, fishing and related activities, and all other hazards and exposures connected with the activities conducted outdoors, do involve Risk. I am fully capable of participating in the activities agreed upon and willingly assume the risk of injury as my responsibility from all activities associated with the adventure, from the use of animals, from adverse weather conditions, from collision with trees, rocks and other man-made or natural obstacle, whether they are obvious or not. Patriot Pursuit inc. and / or their outfitters guides may dismiss a hunter from the field for the day if this policy is not adhered to. THERE ARE NO EXCUSES FOR THE COMPROMISED SAFETY OF THE CLIENT, OR OTHER CLIENTS OR THEIR GUIDES. 2. ALCOHOL / DRUG POLICY: Client will refrain from consuming any alcohol beverage and / or drug during the day or until all firearms associated with the hunt are unloaded and stored for the day. Detection of alcohol and / or drugs use while in the field will be considered grounds for immediate dismissal from said hunter or fisherman. The determination can be made at the sole discretion of Patriot Pursuit inc. or their guides and the associated organization. THERE ARE NO EXCUSES FOR THE COMPROMISED SAFETY OF THE CLIENT, OTHER CLIENTS OR THEIR GUIDES. 3. Be responsible for the game meat, hide, capes and antlers always. 4. Provide all your associated licensing and travel fees before the start of the outdoor activity incurred. 5. Live and hunt in a manner consistent with state and federal; laws and regulations. Abide by the rules provided from time to time by the partnering outfitter / charter and Patriot Pursuit inc. 6. Purchase applicable licenses as necessary for the hunt. 7. Arrange for his / her own travel to and from the outfitter properties except where may be agreed to by Patriot Pursuit inc and our partners. 8. Assume the risk of Hunting, Fishing, Hiking, riding on a quad or side x side vehicle, equine, or any vehicle owned, rented or provided by Patriot Pursuit inc and our partners. 9. I will wear and properly attach my safety harness to the safety line and stay connected the entire time from when I leave the ground until the time, I return to the ground from climbing down. Knowing of the risks (may include specific risks) dangers and rigors required of said activity (s), I certify that I and my family, including minor children or guests, are fully capable of participating in the said activity (s). Therefore, I assume full responsibility for personal injury to myself and to / or members of my family, or for loss or damage of personal property and expenses therefore because of my negligence or the negligence of my family or guest participating in said activity (s). I further understand that Patriot Pursuit inc. reserves the right to refuse any person it judges to be incapable of meeting the rigors and requirements of participating. I FULLY RELEASE Patriot Pursuit inc., their board members and staffers from all liability from any accident, injury or issue that might arise while out on an excursion or on the properties of said individuals and organizations, as well as the providing organization or company.
